University High School Transcripts*

University High School transcript requests must be made in writing. 

Please include the following information in a transcript request:

• Current name
• Any previous names
• Social security number
• Photocopy of drivers license or state ID
• Current address
• Date of birth
• Dates attended the University High School
• The address where the transcripts should be mailed
  
• The request must also include the student's signature. 

There is a $3 charge per transcript.  Transcripts may be requested by:

• Request completed in Special Collections and Archives, Kent Library, Room 306, during normal business hours
• Request faxed to (573) 651-2666 and marked attention "Special Collections".
• Request mailed to Special Collections and Archives, Kent Library, Mail Stop 4600, One University Plaza, Cape Girardeau, Missouri 63701.
